f you submit health-related information through our website, forms, migraine quiz, patient portal, phone, email, or text message, we may use that information to respond to your request, coordinate scheduling, support treatment, payment, or healthcare operations, and communicate with you as permitted by law. The migraine quiz is educational only and does not provide a diagnosis.
